The Rehabilitation Unit (VE) serves all inpatient departments at the Hospital in Ystad. The responsibility includes rehabilitation within specialized outpatient care and specialized day rehabilitation at the hospital. The unit consists of approximately 40 employees from various professions, such as occupational therapists, physiotherapists, social workers, nursing assistants, and nurses. We work in teams with other professionals within the hospital's various departments and clinics. JOB RESPONSIBILITIES

We welcome a physiotherapist to a combined position that includes work in our neurology department and in the patient's home with Early Supported Discharge (ESD).

The departmental work primarily consists of assessment and treatment/rehabilitation of our patients with neurological diseases. We work in a team-based manner with the department staff, conducting daily check-ins and focusing on person-centered care with our patients. Here, you will work closely with a physiotherapy colleague so that you can support each other. We have a goal-oriented approach with our stroke patients and use appropriate treatment and rehabilitation to help the patient achieve their goals. When the patient is discharged from the hospital, these goals follow the patient and are modified as needed, so rehabilitation continues at home.

We plan for an ESD team to start up again in the spring of 2025, where physiotherapists, occupational therapists, nurses, and doctors work multidisciplinary and will be involved in the patient's rehabilitation and care at home. There, we work person-centered with the patient and modify the patient's goals from the department and train with the aim of fulfilling those goals. As a physiotherapist in ESD, you will assess and treat/rehabilitate patients with mild to moderate strokes in their homes, which has proven to be an effective way to improve outcomes and quality of rehabilitation in the early phase. You need to be aware that in the patient's home environment, you will meet and communicate with relatives and be able to be in environments with pets and possible smoke odors.

Both in the work on the department and ESD, you will have contact with municipal and regional primary care to create a safe and secure discharge and further rehabilitation for our patients.

As an employee at a smaller acute hospital, you will have the opportunity for broad learning in many diagnostic areas. We apply a generous approach to further education related to work tasks and actively encourage learning over time. The position includes working with other departments as needed and supervising students.

QUALIFICATIONS

We welcome you who are a licensed physiotherapist with the authority to practice within Swedish healthcare. Your language skills correspond to at least level C1 in Swedish according to the Council of Europe's scale. A B driver's license is also required. Basic computer skills and experience with the Melior journal system are desirable. Experience in stroke/neurology is an advantage.

We are looking for someone who wants to work both in the department and in the patient's home environment. The work is both team-based and independent, which requires you to have the ability to collaborate well and to structure and plan your days, as well as quickly adapt to changing circumstances. We also see that you are adaptable and responsible. Creativity and initiative are assets as rehabilitation must occur according to the patient's needs and situation. We place great importance on personal suitability.
