We are now looking for a nurse or nursing assistant as a care placement coordinator at Psychiatry Southern Stockholm Nacka Hospital!

Qualifications

Licensed nurse, with experience working in psychiatric inpatient care and specialist training in psychiatry is an advantage, or nursing assistant with broad experience from psychiatric inpatient care and must have a protected title as a nursing assistant.

Requirements for nursing assistants:

• Proof of protected professional title for nursing assistants or employer certificate confirming that you had permanent employment as a nursing assistant on July 1, 2023, in accordance with the transitional provisions (2021:739) in the Patient Safety Act (2010:659). If you apply with an employer certificate, you must attach your nursing assistant diploma.

Care placement coordination and its tasks

We are a small workgroup that has close contact and collaboration. As a coordinator, you are jointly responsible for ensuring that patients are coordinated to the right department and assigned a care placement based on psychiatric condition and current care needs. Patient safety is important to us with good reception towards our care neighbors and partners. It is desirable if you have knowledge of our organization, which care providers we collaborate with, and knowledge of patient flows. The workflow can be uneven and sometimes stressful when there is high demand for care placements. Therefore, you need to be able to make decisions quickly and act in accordance with guidelines, routines, and laws. You need to feel comfortable working independently. We will introduce and support you in the work.

The workplace

Our workplace is located in the administrative department at Nacka Hospital near Ektorps centrum with good communication and close to nature. The coordinator function is an on-call service, and the on-call phone is staffed every day of the week, all year round from 08:00 to 22:00, with weekend duty 2 out of 5 weekends.

At Nacka Hospital, we have nine care departments that provide psychiatric inpatient care. Psychiatry Southern also has three other departments located in different geographical locations. The coordinator function is linked to one of the psychiatric care departments at Nacka Hospital where the unit manager will be your closest supervisor. The unit manager and medically responsible doctors are responsible for the daily operation of the coordinator function. As support in our work during on-call hours, we have assistance from responsible backup.
