ÄlSa is a geriatric home visit team for multi-sick elderly patients. The team offers connection to ÄlSa for a limited period for patients with multiple chronic diagnoses, complex care needs, and repeated hospital admissions. The home visit team has a clear goal - to create security and make daily life as good as possible for those enrolled and their relatives. The team consists of nurses and geriatric doctors, and they have a close and well-functioning collaboration. We work daytime, Monday to Friday, and on weekdays without holidays.

We are now looking for a nurse or district nurse to join our team! The position is a permanent full-time employment with a temporary placement at ÄISa for one year.

Nurse/District Nurse for ÄISa team, Örnsköldsvik

As a nurse at ÄlSa, you will conduct home visits either independently or together with a doctor. A significant part of the role involves providing support to patients and relatives through phone contact or home visits.

Your role will include being a cohesive contact between the hospital/health center and home healthcare. You will work with:

• Monitoring vital parameters and taking samples at home
• Assessing symptoms and taking actions
• Administering medications at home
• Following up on actions taken
• Assessing according to Senior Alert (nutrition, pressure sores, fall risk)
• Medication reviews together with doctors

The introduction is individually tailored and takes place over two weeks.

We are looking for someone who

• Is a licensed nurse
• Has good knowledge of Swedish in both spoken and written form
• Has a B driver's license

It is advantageous if you

• Are a trained district nurse or specialist nurse with a focus on elderly care
• Have work experience in elderly care
