Are you a nurse looking to work with specialized care at home? Then Skåne University Hospital (Sus) Hospital at Home is for you. We are looking to expand our team with competent nurses once again.

Hospital at Home is run by the Emergency Care and Internal Medicine department in collaboration with the Infectious Diseases department. This means that patients in need of acute specialized care can be offered this in their own home instead of in a hospital ward.

The inspiration for this care model originally comes from other countries where the concept of "Hospital at Home" is successfully used. Studies show several positive effects for patients cared for at home, such as better eating and sleeping patterns, increased activity, and reduced risk of acute confusion or falls. Experiences so far show very high patient satisfaction and involvement.

JOB RESPONSIBILITIES

We are now looking for you, a nurse who wants to be part of Hospital at Home Lund.

Instead of the patient being admitted to a hospital bed, we as healthcare staff go to the patient's home to provide the care that is currently performed in inpatient wards. Patients can be monitored remotely using digital tools and can always quickly contact us at Hospital at Home and be admitted to the hospital if their condition requires it.

We strive for a broad competence within the team and highly value this as our patients can come from various departments within the hospital, although most are treated for antibiotic-requiring infections. We continuously seek to develop and actively work on which patients we can care for and how we can include and care for as many patients as possible in the best way.

The position primarily involves independent work with patients in their homes where we perform nursing treatments and assessments as well as medical treatments and evaluations. We can manage many of the medical interventions that can be performed in a regular hospital ward. Our patients come directly from the emergency department, from hospital wards, and directly from primary care. It may also involve treatments in the hospital for our patients admitted to Hospital at Home. Some digital work is also included, such as video rounds with patients and coordination of patients within the hospital.

Patients live throughout central Skåne, so you will see a lot while driving or cycling. Working hours are scheduled for day and evening shifts, both weekdays and weekends, without night shifts.

QUALIFICATIONS

We are looking for you who are a licensed nurse with authorization to practice within Swedish healthcare. Language skills in Swedish at least equivalent to level C1 according to the Council of Europe's scale is a requirement. For the job, you need to have previous experience working as a nurse and a valid B driver's license. If you have specialized training in infection, medicine, or similar, it is an advantage. Likewise, if you have experience in internal medicine, infectious diseases, other inpatient care, and/or municipal health and medical care.
