Care Coordinator at Skebäck Health Center

As a care coordinator, you are a central link in our collaboration between primary care, inpatient care, and the municipality. You are responsible for ensuring that each patient receives safety, participation, and influence in the planning of their care and support. In your work:

• you are the patient's main healthcare contact and guide them through the entire care process.
• you call and lead coordinated individual planning (SIP) according to the ViSam model.
• you actively collaborate with the municipality to create a more resource-efficient and person-centered care process.
• you also contribute, if needed, at the district nurse's office with telephone advice, planned consultations, and emergency care.

You work independently but always with the support of experienced and helpful colleagues. In the role, there is also a colleague with the same assignment at 50% of their position.

We are looking for someone who is a licensed nurse or district nurse with experience in care coordination or similar work. You have a professional, warm, and attentive approach, enjoy collaborating, and actively contribute to developing the operations.
