About the Workplace

Vårdcentralen Stadsfjärden is a large healthcare center located in Nyköping's attractive harbor district, operated privately by Praktikertjänst.

Our vision is "tomorrow's primary care today," which means we continuously develop our operations based on our mission from Region Sörmland and our registered population.

We have approximately 17,500 registered patients and are constantly working to develop our methods to provide high-quality care. We strive for high accessibility, where digital and physical care plays a significant role. We handle many of our patient contacts through our digital entry point. The aim is to provide quality care in a medically reasonable time and ensure good continuity for our patients. We also want to create a common understanding among different professional groups and use our resources efficiently.

Collaboration with other professional categories at the healthcare center is a natural part of the work. The workplace is divided into three teams representing all our professional categories. The purpose of the teams' work is for our patients to come well-prepared to the right resource at the right time, based on a common medical assessment.

The team we are currently recruiting for consists of two psychologists, two psychotherapists, and two social workers, many with several years of experience. It is a competent group that enjoys working together.

Job Responsibilities

We work according to Integrated Behavioral Health (IBH), where the work includes high accessibility, stepwise care, collaboration with other professions, and care neighbors. It is important that you have an interest in working according to the principles of IBH, which means prioritizing high accessibility for all residents with mental and behavioral health issues. To meet the needs of the entire patient population, the ability to vary methods and the scope of treatment interventions is required.

Your main responsibilities will be to assess and treat mental and behavioral health issues based on the primary care mission. The focus is on preventive and early interventions based on common reasons for seeking care at a healthcare center. The work is carried out based on the needs of the registered population through physical individual visits, digital healthcare meetings, and internet and group therapy.

Qualifications

We are looking for someone with a degree in social work and a license as a healthcare social worker, as well as CBT level 1.

Experience in primary care is an advantage.
