JOB RESPONSIBILITIES

We are looking for a licensed nurse who wants to be part of our experienced, secure, and dedicated team.

As a nurse coordinator in the municipal primary care, you play a central role in our operations. You work closely with your coordinator colleague.

Your responsibilities include, among other things:

• Planning and coordinating for safe and secure discharge from inpatient care, in team collaboration with other professionals, both within the municipality, region, and private actors.
• Connecting patients to municipal health and medical care based on the medical assignments we receive.
• Being the responsible nurse for patients staying in our short-term department.
• Allocating resources during acute short-term absences based on care load and needs, together with the scheduling manager and unit manager.
• Working on competency enhancement for staff regarding health and medical care tasks and being responsible for our clinical training center (KTC).
• Planning and organizing for overarching initiatives in the operations such as seasonal vaccinations.
• Actively participating in process and development work within the social administration. We are an organization in constant development, where tasks may change and evolve over time.
• Being a resource in the operations and performing occasional home visits as needed.

QUALIFICATIONS

You are an engaged nurse who thrives in a coordinating role where no day is the same.

You should be able to prioritize to ensure patient-safe care.

You see yourself as part of a whole and enjoy working in a team.

You have good experience with IT-based journal systems and can express yourself well in speech and writing in Swedish. We use LifeCare HSL and Cambio Cosmic.

A valid driver's license (B) is required.
