As a SIP coordinator, you will play a central role in creating structure, clarity, and collaboration among the actors surrounding the child.

Welcome to us

We are one of Stockholm's 11 district administrations, with an office in the Slakthus area near Globen. In this role, you will work towards:

  • The commissioning unit for disabilities, where you meet children and young people with disabilities, NPF, or need support according to LSS and SoL.
  • Units within social services, children and youth, where the focus is on social and psychosocial issues, interventions according to SoL and LVU, and support for families.

You will be organizationally placed under the area manager for Children and Youth. The project employment lasts for two years with the possibility of extension.

Your role

You will lead the work with coordinated individual plans (SIP) for children and young people who need interventions from multiple services, such as:

  • Mental health issues
  • Neuropsychiatric disabilities
  • Comorbidity
  • Interventions from both social services and the region

The coordination also includes guardians and networks. You will keep the process together and ensure that the right people are involved, including from:

  • Regional care (BUP, psychiatry, habilitation)
  • Preschool, school, and student health
  • Social services units
  • Other support services such as HVB, outpatient care, and family centers.

You:

  • receive assignments from social workers and assistance officers
  • initiate, lead, and follow up SIP meetings
  • ensure that the right actors participate
  • are responsible for documentation and follow-up
  • provide advice on SIP issues
  • contribute to the development of routines and collaboration structures
  • train internally and externally.

The assignment is followed up annually, among other things through the experiences of the children and guardians, the number of meetings, and identified areas for improvement.

Your skills and experience

We are looking for someone who:

  • Has a degree in social work or another relevant higher education that we consider equivalent.
  • Has good knowledge of SoL, LVU, and LSS legislation and case law
  • Has several years of current experience in social work and a good understanding of social child welfare
  • Is used to leading meetings and creating structure
  • Expresses themselves clearly in speech and writing in Swedish.

We are looking for someone who is communicative, listens, and adapts to the recipient and the situation. You are good at collaborating with others by having an open, accommodating, and problem-solving approach. As a SIP coordinator, you are expected to be self-driven and take responsibility for your work. We believe you act professionally based on your profession by distinguishing between personal and professional matters. You are secure, stable, and have self-awareness. You also demonstrate a holistic view by seeing the bigger picture and considering the broader perspective.
