Welcome to a healthcare center in transition. The Tollarp Health Center with the Degeberga branch is in an exciting phase of development where we are now changing our view on regular healthcare seekers and instead will work in a team-based manner. In interdisciplinary assessments with the patient in focus, the optimized continued treatment/assessment is discussed. A meeting format that both supports patient safety and also develops the individuals within it.
The Tollarp Health Center with the Degeberga branch is located, as the name suggests, in two places, both in Tollarp and Degeberga. We are beautifully located rural centers with about 9000 listed patients at the gateway to Österlen. The Tollarp Health Center acquired the Degeberga branch last year and is in the process of developing opportunities for healthcare seekers at both locations. This allows staff to work at both centers to meet the healthcare needs of our listed patients at both sites. In Tollarp, the health center is conveniently located near the E22 and bus connections to Kristianstad and Malmö, while the Degeberga branch is situated in the village with good bus connections to both Kristianstad and Ystad/Simrishamn.
The staffing at the Tollarp Health Center Degeberga branch is interdisciplinary with a rich exchange of experiences to meet patients' needs. We offer, among other things, doctor consultations, physiotherapy, social work, dietetics, psychology, and occupational therapy. For nurses, specialized clinics are conducted in asthma/COPD, diabetes, elderly care, child health care, and health talks.
JOB RESPONSIBILITIES
We now welcome two nurses or district nurses to join us. The positions entail a permanent employment and a temporary position.
We are looking for someone who is confident and engaged in their professional role. The job includes standard tasks such as reception work, triage, and telephone triage. It is advantageous if you have a specialist education in either Asthma/COPD, Diabetes, or hypertension/heart failure.
We work in three teams with daily rounds, which means that the workgroup jointly assesses and plans for non-urgent patient visits. This contributes to high patient safety and helps us prioritize based on the individual patient's needs.

There is also an opportunity to work in the evening and weekend clinics in Kristianstad, which is included in the assignment at the health center.
The work may involve shifts at both Tollarp and Degeberga.
QUALIFICATIONS
We welcome you who are a licensed nurse with the authority to practice within Swedish healthcare. If you are a trained district nurse, this is highly meritorious. Language skills in Swedish at least corresponding to level C1 according to the Council of Europe’s level scale is a requirement. Experience from primary care and specialist competence in asthma/COPD, diabetes, or hypertension/heart failure is also meritorious.
You as a person are service-oriented and your work is characterized by patient focus. We also value high social competence and good collaboration skills. You have a heart for primary care and enjoy working in interdisciplinary teams. Furthermore, you are attentive and driven to provide good and qualitative care. Additionally, you are meticulous, structured, and have a flexible approach. Great emphasis is placed on personal suitability.
