The unit for palliative care and advanced home healthcare (ASIH) within Region Skåne aims to provide equal care to all residents of Skåne, regardless of diagnosis, age, or residence. In Kristianstad, the unit has a specific mission to provide palliative specialist care through home visits around the clock. The unit consists of several parts that work closely together; palliative outpatient clinic, palliative care department, and advanced home healthcare. In our palliative care department in Kristianstad, we have ten care places, and within the municipalities of Kristianstad, Bromölla, and Östra Göinge, we have about 32 care places in the home. We collaborate with municipal care and support to offer comprehensive care together with them.

Patients come to our palliative care department for shorter or longer stays, depending on the reason for admission. We also have a palliative consultation team that visits referred patients in hospitals and home environments and provides support to other healthcare providers. We also have a palliative specialist clinic (PM) during the day. This, together with our consultative support, will constitute a significant part of our future mission as we actively work with the future of healthcare by meeting more patients at an early stage. One step in this is Basoncology, which is a region-wide initiative that brings care closer to patients. Individuals who have undergone cancer treatment are offered symptom relief and some treatment at home instead of being hospitalized.

JOB RESPONSIBILITIES

We now welcome a new colleague who has a genuine interest in palliative care and wants to develop in the field with us at ASIH in Kristianstad. With us, you will be offered a developing and exciting job where you become part of a multidisciplinary team.

As a nurse with us, you will perform advanced healthcare in the home. You will lead and actively participate in planning around the patient's daily care together with other professionals and collaborate with other healthcare providers. You are also expected to actively participate in the development and improvement work of the organization. The goal of specialized care is to collaborate with the patient and other healthcare providers to alleviate the symptoms that the patient is troubled by to achieve the highest possible quality of life. This means addressing the patient's physical, psychological, and existential needs, as well as providing support to their relatives.

As a newly hired nurse with us, you will receive at least four weeks of shadowing. We follow an introduction program developed at the unit, and after the shadowing is completed, you will receive mentorship from a colleague based on your needs. We value development work, continuing education, guidance, and reflection for both new hires and experienced staff. We have close collaboration with the palliative care department for inpatients, ASIH in Hässleholm, as well as with the hospital and various clinics. We have a good and close collaboration with CSK regarding patients. With us, you will also be offered wellness grants and individual scheduling through MultiAcsess.

QUALIFICATIONS

We are looking for a licensed nurse with the authority to practice within Swedish healthcare, preferably with a few years of professional experience. Language skills in Swedish equivalent to at least level C1 according to the Council of Europe's scale are required. We would also like you to have specialist training in oncology and/or palliative care. Since the operation consists of care places in the patient's own home or in special housing, a valid B driver's license is a requirement for the position.

Part of the work involves support and conversations, so it is important that you are communicative and have the ability to meet people on different levels. Furthermore, you have a flexible approach and can prioritize between different tasks when the work pace is high. This includes mobile teams and specialized palliative care, outpatient services within close care, acute care within close care, 1177 by phone, and some region-wide functions. In addition, the department is responsible for the development and follow-up of the transition to close care in Region Skåne. The number of employees is expected to be around 1,000.
