The Social Administration is responsible for care of the elderly and people with disabilities, as well as support, counseling, and other services for children, youth, and adults.

Description

The Municipal Rehab is organized within the Social Administration of Rättvik municipality.

Within the unit, there is also a service that supports recently discharged patients from the hospital according to the SUS law. Here, there is also housing coordination, a dietitian, and a short-term department. The main mission of the unit is to contribute to the municipality's goal of making residents as independent and active as possible by motivating and supporting patients to maintain and develop their active abilities. The target group includes young adults, adults, and the elderly in regular housing, special housing for the elderly (SÄBO), and group housing under LSS.

The work is characterized by commitment, innovation, development, and customer focus.

Currently, the rehab team consists of 4 occupational therapists and 5 physiotherapists. Since one of our licensed physiotherapists has chosen to further their education to meet the increasing age curve, we are now looking for a replacement to start as agreed.

Job Description

As a licensed physiotherapist, you will be responsible for assessment, rehabilitation, prescriptions, delegation to nursing staff, and the provision of assistive devices. We value interdisciplinary teamwork so that our patients feel secure and involved in their daily lives and to create opportunities for them to remain at home.

In Rättvik municipality, we work with Safe Homecoming after hospital stays, which means quick and more intensive interventions together with nursing assistants linked to rehab and home care upon the patient's return from the hospital.

Your main tasks as a physiotherapist will include conducting mobility assessments and evaluating physical conditions for training. You will design individual training programs and instruct/train nursing staff in everyday rehabilitation and mobility. Prescribing assistive devices is part of the job, as well as participating in the municipality's work in fall prevention, preventive, and developmental work. We document in Lifecare HSL.

Qualifications

We are looking for a licensed physiotherapist. It is advantageous if you have good knowledge of regional and municipal health and medical care, as well as previous experience in multidisciplinary teamwork. We also appreciate an interest in monitoring and development within the healthcare and social care sector. Since communication and documentation are part of the tasks, we want you to have digital competence and master the Swedish language in both speech and writing. As the work involves home visits throughout our vast municipality, you need a valid driver's license for a passenger car.

As a person, you have good collaboration skills, and you can motivate and inspire your surroundings with your rehabilitative/enabling perspective. You enjoy working both independently and in a team.
