We are now looking for a case officer on behalf of our client. Typical tasks include investigation/decision-making according to SoL and LSS, as well as follow-ups, planning for patients discharged from hospitals and homes, or to short-term/special accommodations.

Planning is done via Life Care (a joint system between the municipality and the region), collaboration meetings, and home visits. Documentation is carried out in the operational system Pulsen Combine.

Full-time on-site, some remote work is possible in consultation with the client. Starting as soon as possible, the assignment extends until February 28 with the possibility of extension.
