We are now looking for a dedicated Home Care Coordinator for the Fristad unit!

The Health and Elderly Administration in Borås City works towards the goal that both younger and older residents in need of care, support, and service, as well as health care and preventive health work, should have a good life every day. The administration consists of about 2,500 employees, of which approximately 700 work within the municipal home care.

We who work in home care Fristad are a wonderful team consisting of nursing assistants, caregivers, service assistants, managers, coordinators, and home care coordinators. With us, men and women from different backgrounds work, enriching our operations and broadening our competence.

Home care Fristad is based in large and nice premises centrally located in Fristad. There are also good parking opportunities for you as an employee.

Home Care Coordinator

Job Responsibilities

As a home care coordinator, your main task is to plan and schedule ordered services from case managers and health care professionals. You work towards set goals and strategies and have close cooperation with the unit managers in home care as well as several other professional roles. Your task is to optimize scheduling in a resource-efficient manner, which includes finding logical solutions for service planning with the user in focus. You have daily communication with SÄS where you monitor users' discharge and return home.

The home care coordinator is also responsible for handling staffing during short-term absences and, if necessary, rescheduling service schedules.

The home care coordinator works in systems such as TES, SAMSA, Time Care pool, VIVA, etc. Daily use of Excel, Word, and communication via email and phone.

Working hours are scheduled during weekdays with occasional weekend shifts.

The employment is a temporary position from June 2nd to August 15th.

Qualifications

You should have post-secondary education in administration or at least one year of documented experience in planning home care services. If you have worked as a nursing assistant in home care, it is an advantage.

We are looking for someone with good analytical and logistical skills. You have a commitment and a desire to develop and improve planning based on set goals within the Health and Elderly Administration. You should have good communication skills in both spoken and written form, as well as good collaboration skills and a high sense of service.

The work may involve rapid changes and requires flexibility in your working methods, where you should be able to handle multiple tasks simultaneously. You should also have good computer skills and the ability to manage various operational systems and the Office package.

You need to understand the relationship between the different components that govern home care, namely orders based on the Social Services Act (SOL) and the Health and Medical Services Act (HSL). You should also have an understanding of the relationship between the unit's needs, staff, and finances.

It is important that you are professional and can make decisions based on your responsibility role.

Great emphasis will be placed on personal suitability.

A B driver's license is required for the position.
