We are now looking for a registered nurse to join our team in Innerstan (City Centre), based out of our office at Sabbatsberg Hospital.

As a nurse, you are responsible for both planned and urgent home visits to patients in Innerstan. You conduct medical assessments, plan and coordinate interventions, and collaborate with health centers, other healthcare providers, and our call center.

The role gives you great opportunities to use your clinical experience and make independent decisions, while always having the support of an experienced team.

You work according to a rolling schedule with day, evening, and weekend shifts (2 weekends out of 5). On weekdays during the day, you work from our office at Sabbatsberg Hospital where you assess incoming referrals, coordinate care efforts, and ensure that patients receive the right care at the right time.

Aleris Basal Home Care provides care on behalf of health centers in Stockholm County and offers medical interventions in the patient's home when health centers are unable to make home visits.

We are part of Aleris Primary Care, which also includes ASIH, specialized palliative care, Rehab Station, and Uppsala Urgent Care.

• Is a registered nurse

• Has at least two years of experience in the profession

• Holds a valid driver's license (Category B)

• Has experience with TakeCare and good computer skills

• Fluently speaks and writes Swedish

Experience from home care, primary care, or emergency care is an advantage.
