We are now looking for an Assistance Case Manager responsible for discharge processes!
The care services in Falun Municipality are undergoing an exciting transition towards Good and Close Care and a new Social Services Act. We are strengthening the team with a responsible case manager for discharge processes – a key role for you who want to combine operational work with development and collaboration.
As an Assistance Case Manager, you will be part of our planning team and work with discharge planning from inpatient care and short-term housing to ordinary housing. The role combines operational work with overall responsibility for developing routines, processes, and collaboration together with the unit manager and section manager.
Job Responsibilities
You will become a central part of our established planning team and have overall responsibility for the discharge processes. You will work both close to the individual and strategically to develop working methods that make a real difference.
In this position, you will:
- Be responsible for and coordinate discharge planning in the team
- Plan discharges from hospitals and short-term care
- Ensure quality and competence in the work
- Develop and improve working methods and processes
- Strengthen collaboration with the region, health centers, and other actors

- Reduced care times and prevention of readmissions
- Creating a cohesive, secure, and person-centered care chain
Qualifications
For the role, the following is required:
- Several years of experience in public administration with a focus on the discharge process
- Experience in leading and instructing colleagues in working methods

Terms of Employment
Permanent full-time employment with start date as agreed.
