We are looking for a Social Worker/Case Manager for a consultancy assignment within public administration, focusing on elderly care and the Support and Service Act for Persons with Certain Functional Impairments (LSS).

In this role, you will handle cases according to the Social Services Act (SoL) and LSS, including investigations, follow-ups, and reviews. The work also involves planning for patients ready for discharge and collaborating with healthcare and social care providers to ensure appropriate interventions and a safe return home.

We are looking for someone who:

• Is a Social Worker or has other education considered equivalent, such as a behavioral scientist
• Has experience with public administration within the LSS framework
• Has experience in case handling according to SoL and/or elderly care
• Is proficient in documentation and independent case handling
• Has experience with business systems such as Lifecare or similar
• Has good proficiency in Swedish, both verbal and written
• Has a valid driver's license (Category B)

About the assignment:

• Period: September 1, 2026 – October 31, 2026
• Scope: Full-time
• Location: Contact us for more information regarding the workplace
• The consultant is expected to be on-site at the facility every other week
