Andersberg Health Centre currently has approximately 45 employees and provides primary care services including a doctor's clinic, district nurse clinic, Child Health Centre (BVC), psychosocial team, occupational therapy, physiotherapy, and home visits in a multicultural area near Gävle city centre.

Job Responsibilities

As a Rehabilitation Coordinator, you are responsible for coordinating interventions for sick-listed patients who need support in their work-oriented rehabilitation. You play a central role in creating structure and momentum in the sick leave and rehabilitation process. You collaborate closely with doctors, other healthcare professionals, patients, and the unit manager, serving as a key support throughout the process. You also serve as the unit's contact person for relevant stakeholders in the sick leave process. The majority of your work involves handling sick leave cases, but you also actively contribute to disseminating knowledge about the region's development work within the sick leave process.

As a Rehabilitation Coordinator with us, you will, among other things:

• Support sick-listed patients in the process of returning to work or entering the labor market.
• Conduct assessment interviews with patients and contribute to the creation and follow-up of rehabilitation plans.
• Monitor sick leave statistics.
• Coordinate the unit's Trisamteam, which is a structural collaboration between the Swedish Public Employment Service, the Social Insurance Agency, the municipality, and healthcare.

Qualifications

We are looking for someone who is flexible, responsible, and solution-focused. The role requires the ability to work independently and take initiative to develop the position. You are receptive and adapt your communication to the recipient.

As a Rehabilitation Coordinator, you must:

• Hold a university degree in medicine, psychology, or social work, such as a social worker or behavioral scientist.

For this position, you should have in-depth knowledge within the rehabilitation field and experience collaborating with both internal and external stakeholders. Previous professional experience from healthcare and/or insurance medicine is meritorious. You should have basic knowledge of medical terminology and experience reading and documenting in journals or similar systems.
