Your responsibilities

The Home Care Administration is looking for a registered nurse, to be placed on the planning team for rehabilitation, care, and health (PROV), which collaborates with Region Halland to ensure patients' safe transition home from Halland Hospital.

PROV is a cross-professional team consisting of social secretaries, nurses, occupational therapists, and physiotherapists. The planning team is based at Skolgatan.

The task of PROV is to facilitate and streamline the care flow from inpatient care to municipal home care through clear care planning. The patient should experience a safe and secure transition to other care and support services when discharged from Halland Hospital. Care planning is conducted almost exclusively digitally via TEAMS.

In the event of a lower workload at PROV, the position may be performed at another unit within the administration.

Currently, over 100 nurses work in our round-the-clock home care operations. We are proud of our elderly care services and can offer exciting and varied work.

Your qualifications

We are looking for you who are a registered nurse and have broad experience working in healthcare. As a person, you are responsible and motivated by organizing work together with your colleagues in the best possible way. Knowledge/experience from Halmstad Municipality's home care is an advantage.

You should have the courage and desire to work both independently and together with the professional team and other actors. To thrive with us, we believe you are challenged by situations where new thinking and creativity are needed.

The job also requires good computer skills as it involves documentation and planning in the systems and tools we use.

A driver's license for a passenger car with a manual transmission is required for the position.

Great importance will be attached to personal suitability.
