Specialist Physician for Mobile Healthcare Team in Gävle

The Mobile Healthcare Team provides home care for patients who have difficulty visiting their regular healthcare provider. The team is organizationally part of VO Primary Care in Region Gävleborg. We work broadly and collaborate with all health centers in the area, both company-run and privately run, as well as with municipal healthcare and operations at Gävle Hospital. Doctors and nurses work together in the team to ensure professional and secure care. The operation is based on a team vehicle that operates in Gävle Monday–Friday to enable close and coordinated care interventions for our patients.

Job Responsibilities

The work of the Mobile Healthcare Team is based on a holistic assessment of the patient's situation in their home environment. Many of our patients are elderly and multi-morbid. Performing assessment and treatment in the home environment provides significant added value for the patient, and as healthcare providers, we meet the whole person's life conditions, which gives us invaluable guidance in our work. A large part of this work involves maintaining and developing collaboration with primary and secondary care as well as various municipal actors. Our work also includes following people during their final time in life.

As a specialist physician with us, you will among other things:

• Perform urgent and planned assessments at home.
• Carry out planned treatments at home.
• Collaborate with various healthcare providers.

Qualifications

We are looking for you who have good collaboration skills and enjoy working in a team together with patients, relatives, and other professions. You are confident in your professional role and have an empathetic and professional approach. You are flexible in your working style and adapt easily to changing situations and the needs of the operation. At the same time, you work systematically and have a good ability to plan, prioritize, and organize your work efficiently. You are proactive and are happy to contribute solutions that create added value for patients. You see opportunities in change and actively contribute to developing methods and improvements that strengthen care and operations.

In the role of specialist physician, you must:

• Be a licensed specialist physician with authorization to work within Swedish healthcare.
• Hold a valid Category B driver's license.
• Have very good knowledge of Swedish, both spoken and written.

It is meritorious if you have specialist competence in general practice or another relevant specialty, such as geriatrics, internal medicine, cardiology, or infectious diseases.
