Our 1,800 employees work with primary care and health in Uppsala County. We offer a wide range of services within primary care. Our patients meet us at Region Uppsala's own health centres or within habilitation services, and we provide advice by phone via 1177. Education, research, and development are important to us, as we tailor our offerings to the needs of the residents. We already offer mobile teams, video visits, and smart e-services.
A unique opportunity to combine roles
You are warmly welcome to apply for the position of Counselor with us! We offer a permanent position at 40% where you will work as an Elder Coordinator. Additionally, there is an opportunity to temporarily increase the workload up to 100% as a Counselor at the health centre.

Your assignment
In the role of Elder Coordinator, you will work at our elderly clinic with the aim of creating security, continuity, and quality in care for the elderly with increased care needs. You will work together with two other Elder Coordinators and a multidisciplinary team. The work involves early identification of people over 75 years old at risk of significant care needs, preventive work through health talks, and coordinating care interventions both within the health centre and with other actors. The assignment also includes planning, following up, and developing interventions based on a structured patient process. There is a well-developed induction program for Elder Coordinators.
As one of our counselors is on leave, there is an opportunity to temporarily increase the workload to 100% by working as a Counselor at the health centre. The work as a Counselor involves meeting and treating people in crisis, individual psychosocial counseling, assessment, support talks, and treatment for adults over 18. There is scope for group activities. You will be part of a psychosocial team together with a counselor and two psychologists. You will contribute to developing operations and collaboration methods, both within and between health centres and with our care partners. Your consultation to colleagues at the health centre is an important part of the work, and multidisciplinary working methods are central to us.
Your qualifications
We are looking for you who are a licensed Health and Social Care Counselor with good experience in psychosocial work. We would also like you to have basic psychotherapy training (step 1) as well as knowledge of insurance and rehabilitation medicine.
As a person, you are good at planning, organizing, and prioritizing work effectively. The work requires you to be flexible in your approach and have strong initiative. The job assumes that you have good collaboration skills and enjoy working in a team, but also that you have the ability to work independently and lead yourself. You enjoy developing operational and collaboration methods, both within and between health centres and our care partners. We place great emphasis on personal suitability.

Samariterhemmet Health Centre is located in central Uppsala, a stone's throw from the Travel Centre. Our health centre is a large operation with 14,000 registered patients. Together we are over 80 employees, and all professional areas within the health centre's mandate are represented. The broad variation of patients creates a developing workplace.
