The health centres in Matfors and Stöde are part of the healthcare area Sundsvall/Ånge, which also includes Liden, Granlo, and Ånge health centres, with which we have continuous cooperation. All units work based on the concept of "close care," where patient needs are at the center. To keep up with the development of healthcare, we also offer digital care. As a counselor, you will be an important part of our psychosocial team and will have close and continuous cooperation with other staff at the health centre. The introduction process is conducted in consultation with you, depending on previous experience with the role.

We are now looking for a substitute with a start date of 2026-08-15 until 2028-01-15. The role involves approximately 50% working time at Matfors health centre and approximately 30% working time at Stöde health centre. We are a tight-knit team that knows our patients well and has a cozy and family-like atmosphere that is contagious. Do you want to become part of that team? As a counselor in primary care, you will work with standard counseling tasks. The role includes, among other things:

• Psychosocial treatment work.
• Assessments and shorter treatment contacts.
• Support and crisis conversations.
• Collaboration with other professional groups.
• Cooperation with external actors when necessary.

• Are a licensed health and social care counselor.
• Have at least two years of work experience in primary care.
• Have Swedish language skills in speech and writing at level C1 or higher according to the Council of Europe's scale.
• Hold a valid B driving license.

• Flexible.
• Team player.
