Job Description

We are now looking for a Health and Social Care Counselor to join our growing operations. The position is for 50% and the work is performed on-site at the health center.

As a Health and Social Care Counselor with us, you will work with patients experiencing varying degrees of mental and social ill health. Your work is based on a holistic perspective where the patient's needs are at the center. Your main tasks include assessment, counseling, supportive conversations, and shorter treatment interventions within the scope of primary care.

You will work closely with other professions at the health center and be an important part of our interdisciplinary team. The role also involves collaboration with the municipality, social services, the Swedish Social Insurance Agency, and other healthcare providers when the patient's situation requires it. You will contribute your psychosocial competence to the team's work and actively participate in the organization's quality and development work.

Qualifications

We are looking for you who are educated as a social worker and have further education within health and social care, for example through the Health and Social Care Counselor program. Experience working within health and social care is required.

We wish that you, as an applicant, are confident both personally and professionally and have a patient-focused approach. You have a good ability to work independently while appreciating collaboration with other professional groups.

It is an advantage if you have experience working within primary care and knowledge of collaboration with the municipality and other external actors. Experience in counseling, rehabilitation work, and work with sick leave processes is also considered an advantage. Multilingualism (preferably in Arabic) is seen as an asset but is not a requirement.
