Habilitering is specialized healthcare for adults with autism, intellectual disability, mobility impairment, or acquired brain injury. We provide health-promoting and treatment interventions based on each individual's unique needs and conditions, and possess specialist knowledge of how disabilities can affect daily life, health, and development.

Description

Here you will have the opportunity to work as part of an interdisciplinary team to provide interventions for adults within Habilitering's target groups. In the team, you will contribute important pieces based on your professional expertise. The specialized interventions are provided based on health problems related to the patient's disability that cause significant difficulties in daily life.

The catchment area for Habilitering Borås includes the municipalities in Södra Älvsborg, and the workplace is located in Borås. The work involves travel to our clinic in Alingsås.

Job Responsibilities

The nurse's responsibilities include investigating and assessing health problems based on the patient's disability within their area of competence, as well as planning and implementing interventions to give the patient increased opportunities to improve and/or maintain their health. All interventions are planned together with the patient and, if necessary, their network.

Tasks are related to issues such as healthy lifestyle habits, pain, nutrition, breathing/circulation, skin/tissue, elimination, and pain. Consultation with the patient/network is a recurring activity. You will also work with follow-up according to the national quality register for patients with spinal cord injury, follow-up of patients with multiple disabilities, and patients with neuromuscular diseases. Collaboration with colleagues in regional and municipal primary care, as well as with colleagues in other medical specialties, is a natural part of your duties. You often coordinate care contacts the patient already has or is in need of.

Interventions can be given individually or in groups during limited periods. Digital visits are conducted when possible. The work takes place primarily in the facility's premises and as needed in the patient's immediate environment.

We are looking for you who

• Are a registered nurse with specialist education in open healthcare. It is an advantage if you have several years of professional experience, experience in regional/municipal primary care, and experience working in interdisciplinary teams.
• Have experience, interest, and knowledge about working with people in our target group. Further education in psychiatry or equivalent is an advantage.
• Are responsible and feel confident working both in a team and independently.
• Are flexible, solution-focused, have the ability to structure your work, and have good collaboration skills.
• Hold a B-driver's license as the work involves travel.
