Your Assignment

Common tasks in this assignment include providing support in daily life, as well as offering, motivating, and enabling participation in activities, social community, and meaningful occupation.

You will work to promote a good quality of life and create conditions for the individual to develop their abilities and strengthen their resources. This is done through a pedagogical, motivating, and positive approach, where you encourage, stimulate, and engage to increase independence.

Together with your colleagues, you are responsible for ensuring that the users' assessed support needs are met. The assignment also includes being a contact person for certain users, which entails a special responsibility for planning, coordination, and follow-up of measures. As a contact person, you establish, follow up, and revise implementation plans, as well as acting as an important link between users, relatives, and other professionals.

The work involves acting independently in the meeting with the individual, while maintaining close collaboration with colleagues, the unit manager, and other partners. Cooperation with other operations in the area is an important part of ensuring coordinated and qualitative support.

You are expected to contribute to a good working environment, be well acquainted with the organization's guidelines, routines, and goals, and actively contribute to developing and maintaining high quality in the operations.

The work involves documentation and journaling.

Working hours are scheduled for days, evenings, and weekends.

Description of the Workplace

Residential Support is part of the Functional Support area within the Welfare sector. The people who receive support from us live in their own homes around Söderhamn. Residential Support operates throughout the municipality, and our common office is located in central Söderhamn.

Residential Support is a practical, pedagogical, and social intervention aimed at strengthening the individual's ability to manage daily life – both at home and in society. We carry out interventions based on granted assistance, where the work is based on the individual's own resources, needs, and wishes. Interventions are planned, implemented, and followed up through implementation plans.

Your Competence

We are looking for you who are an educated assistant nurse/caregiver, social pedagogue, or have other education that the employer considers equivalent.

It is an asset if you have experience working with target groups within LSS (Act on Support and Service for Persons with Certain Functional Impairments), mental illness, and/or addiction problems. Experience working with people with mental disabilities and neurodevelopmental variations is considered an advantage. It is also an asset if you have education in Case Management (CM) as well as knowledge of and experience working with low-affective approach, pedagogical approach (PFA), An Independent Life (ESL), and motivational interviewing (MI).

As a person, you have high personal maturity and are secure, stable, and have good self-insight. You have an empathetic ability and find it easy to put yourself in others' perspectives and situations. You are flexible, attentive, and have the ability to adapt your working method based on the individual's needs and conditions.

You have a professional approach, can distinguish between the personal and the professional, and create trusting relationships. You are attentive to details while having the ability to see the big picture, understand your role in the assignment, and contribute to the best interests of the organization.

The work requires documentation and collaboration with various parties, which means you need to have a good ability to express yourself in Swedish, both verbally and in writing.

A B-class driver's license is a requirement for the position.
