Osteoporosis Nurse to the Osteoporosis Unit Gävleborg

As an osteoporosis nurse, you will be part of the Osteoporosis Unit Gävleborg, which is a regional unit. The unit is newly established, but the patient flow has been developed previously, and we now see an opportunity for further development and improvements with the new unit. The unit consists of six osteoporosis nurses and three osteoporosis coordinators, one of whom is a process leader.

JOB RESPONSIBILITIES

As an osteoporosis nurse with us, you will have an independent and varied job where you meet patients who are being investigated and followed up for suspected osteoporosis after fractures. Patient contact primarily occurs digitally or via phone, which means you will work with patients from across the region.

You will be responsible for your assigned health centers, while collaboration across unit boundaries occurs as needed. You will work closely with six colleagues located in different areas of the region and serve as an important link between the osteoporosis coordinator at the hospital and primary care.

The work is characterized by a high degree of independence and personal responsibility for monitoring and follow-up.

As an osteoporosis nurse with us, you will work with:

• patient contact via digital meetings and phone calls
• responsibility for patients from the entire region
• working with assigned health centers
• collaborating with colleagues in both primary care, hospitals, and municipalities
• investigating patients with suspected osteoporosis after fractures
• collaborating with family doctors regarding treatment
• independently managing monitoring and follow-up of patient cases.

QUALIFICATIONS

We are looking for a flexible and committed person who enjoys working closely with several professions in a varied everyday life. You have an interest in development and improvement work, are confident in your professional role, and have the ability to meet people in a professional and empathetic manner.

In the role of osteoporosis nurse, you should:

• be a district nurse or a licensed nurse with relevant experience
• have a valid B driver's license.

For this position, it is advantageous if you have experience working in primary care and/or if you have in-depth knowledge of osteoporosis.
