Working as a Counselor and Rehabilitation Coordinator at Aliso Health Center Farsta involves a central role where you act as a link between healthcare and other societal actors. You contribute a psychosocial perspective to patient work, combined with medical and psychological competence. You are part of an interdisciplinary team where you provide knowledge about patient rights and opportunities for support measures in society.

Main Responsibilities:

  • Assess the patient's social and economic situation and identify needs for support and interventions.
  • Coordinate interventions and collaborate with relevant actors such as healthcare neighbors, social services, the Swedish Social Insurance Agency (Försäkringskassan), and the Public Employment Service (Arbetsförmedlingen).
  • Convene and lead network meetings and SIP meetings (Individual Rehabilitation Plan).
  • Support sick-listed patients in the rehabilitation process and during return to or entry into working life.
  • Collaborate with actors linked to the rehabilitation process.
  • Provide consultative support on social issues to other professions within the organization.

About You:

  • Education as a Social Worker (Socionom).
  • Experience in a similar role is an asset.
  • Knowledge of the TakeCare journal system is an asset.
  • As a person, you are flexible, professional, and have good collaboration skills. You have the ability to work independently, take responsibility for individual cases, and plan and organize your work.
  • You actively contribute to a good working climate and value a good social work environment as well as a strong team spirit.
