Health and Medical Care is part of the Social Services Department, which also includes Elderly Care, Support for Persons with Disabilities, Individual and Family Care, as well as Administration and Digitalization. We are the municipality's largest department with approximately 1,200 employees.

Health and Medical Care in Strängnäs is responsible for healthcare and rehabilitation for the municipality's residents. Here, nurses, physiotherapists, occupational therapists, and assistant nurses work together with the patient at the center. Our main tasks are to provide care and treatment to individuals who need support and cannot reach their local health center.

We are currently seeking a Care Manager/Nurse for a one-year project position.

RESPONSIBILITIES

The Care Manager serves as a supervisory and coordinating function within municipal health and medical care. The assignment aims to contribute to safe, cohesive, and patient-safe care, as well as strengthen collaboration between professions, units, and external actors. The Care Manager complements the licensed nurse's core mission by promoting structure, continuity, quality, and development within the operational area.

The Care Manager assignment is estimated to be approximately 75%, with the remaining 25% being clinical work within the unit. The Care Manager has overall responsibility for:

  • Supporting daily coordination and prioritization within teams in the operational patient care work.
  • Contributing to good quality, regulatory compliance, and patient safety.
  • Contributing to the implementation of the "Close Care" arrangement (in collaboration with the new Social Services Act) in daily work.
  • Participating in internal and external collaboration.
  • Monitoring and supporting work with routines, processes, and working methods.
  • Contributing to education, mentoring, and competence development.
  • Ensuring good information flow between relevant actors.

QUALIFICATIONS

We are looking for someone who is/has:

  • A licensed nurse.
  • Previous experience working in Health and Medical Care within a municipal operation.
  • Good ability to express yourself in Swedish, both orally and in writing.
  • Good computer skills and proficiency with the entire Office package.
  • A Class B driver's license.

It is an advantage if you have:

  • Experience in a supervisory position.
  • Experience working with digitalization.
