The Role of the Social Worker

We are now looking for a social worker for an exciting assignment within specialist healthcare where you will meet people who have faced significant changes in their lives. As a social worker with us, you will work to map and treat patients' psychosocial situations together with other team members. This involves collaboration and cooperation with other professions and organizations. A large part of the position also involves working with patients' work rehabilitation through rehabilitation coordination. There you will become part of the region's network for rehabilitation coordinators.

Responsibilities include:

• Crisis and support conversations
• Support for relatives
• Coordinate patients' work rehabilitation
• Community information
• Collaboration with healthcare staff, other rehabilitation professionals, authorities, and providers
• Participation in development and quality work

We work with digital solutions and digital patient meetings, which we will continue to develop, so we would like to see that you have an interest in digital treatment methods.

The position is a permanent full-time employment. Working hours are during the day on weekdays, and the base location is at Ryhov County Hospital in Jönköping. Since the Rehabilitation Medicine Clinic is a county clinic, it also includes working with the team in Värnamo. Travel between Jönköping and Värnamo is done during working hours.

Your Profile

To be considered for the position, a degree in social work is required. If you are a licensed healthcare social worker, it is an advantage. Since travel within the county is included in the position, a driver's license (B) is required.

We also view positively if you have:

• worked within the framework of SoL (Social Services Act) and LSS (Act on Support and Service for Certain Disabled Persons) with case management, or have good knowledge in the area.
• knowledge of the social insurance system
• previous experience as a healthcare social worker
