The home support team consists of 31 employees – case managers, nurses, occupational therapists, physiotherapists, and nursing assistants – who work closely together to create safe and cohesive care for our patients. We also have a close collaboration with Region Skåne, which gives us good conditions to provide the best possible support.

As a physiotherapist with us, you will work evidence-based with rehabilitation in the home environment and be responsible for prescribing aids. You will guide and support care staff in a rehabilitative approach. You will be involved in the work with coordinated individual plans and participate in planning meetings together with case managers in connection with discharge from the hospital. There, you contribute your knowledge to ensure a safe, clear, and well-planned transition from hospital to home. You will also work in our shared IT system, Mina planer, where collaboration and a holistic view are in focus.

Who are we looking for?

To succeed in the role, we see that your personal qualities are of great importance, meaning that you have a good ability to collaborate and communicate and promote a good work environment. You can motivate and inspire those around you and have the ability to work both independently and in a team. We want you to be able to prioritize and plan your work while having the ability to quickly adapt to the needs of the operations for the day. You have the ability to collaborate with different professions and work in processes to create a team spirit. A holistic view and a rehabilitative perspective are also important qualities for the job.

You are a licensed physiotherapist. For us, it is an advantage if you have good knowledge of regional and/or municipal health care. You who apply to us have a B driver's license for manual transmission to easily get around our municipality when needed.

The Social Services Administration's operations target the elderly and people who need functional support as well as those in need of home health care. The operations are functionally divided into four areas: support in ordinary housing, support in care and nursing homes, health care, and functional support. The administration also includes the authority unit, the national gymnasium, and the aids center in Eastern Skåne.
