Within the social care administration, about 1200 dedicated employees work to provide support and care to those in need in our municipality. The work takes place within elderly care, home care, disability services, social psychiatry, and individual and family care.

The health and medical care unit consists of nurses, occupational therapists, physiotherapists, rehabilitation assistants, and HSL-USK. We work with patients aged 18 and above who need health and medical care at the primary care level in their homes, both in special and ordinary housing.

JOB RESPONSIBILITIES

In the role of coordinator, you play an important function in the daily operations. A large part of the workday is characterized by close contact with nursing staff via phone and various digital messaging functions. Through triaging and professional assessments, you contribute to ensuring that the right intervention is provided at the right time, with the patient's best interests in focus.

The work also includes administrative tasks and provides support to your nursing colleagues in their daily work. Together with another coordinator and patient-responsible nurses, you are responsible for structuring and planning the workday in an efficient and well-functioning manner. Collaboration and good communication are central parts of the role to create safe and quality care.

The coordinator role is independent and involves a great deal of personal responsibility. You are expected to be able to make decisions, prioritize, and work solution-oriented, while also being a stable and accessible contact person for the operations. The role offers variety, development, and the opportunity to make a real difference in the everyday lives of both patients and employees.

QUALIFICATIONS

We are looking for someone who meets the following requirements:

• Licensed nurse with experience in home healthcare
• Good communication skills in Swedish, both spoken and written
• Good computer skills and comfort in working with digital systems
• Experience working with Mina Planer, NPÖ, Pascal, and LifeCare
