Our Offer

Primary Care Center Östergötland consists of 33 region-operated health centers, Vårdcentrum Finspång, Child Health Care Unit, 1177 and emergency centers in the evening and on weekends, and a General Medicine Training Center.

We conduct a broad primary care operation where we offer care to everyone throughout their lives; newborns, children, adolescents, adults, the elderly, and patients in the end stages of life.

Vårdcentralen Finspång has about 20,800 registered patients in a mixed population and approximately 100 permanent employees. In addition to the basic assignments for the health center, the operation also includes an emergency center, a branch in Rejmyre, and conducts some clinical activities. The health center is in a development phase where staffing is being strengthened and operational development is being carried out in many areas.

Since 2025, Vårdcentralerna Skärblacka and Finspång have been a joint unit. The operation has about 130 employees in total and collaborates today at both management and employee levels.

Job Description

You will primarily work in our team for our elderly care reception. The team includes doctors, nurses, and dietitians. There, together with the team, you will provide specialized care for frail elderly and people with complex, unstable multimorbidity. You are the patient's primary care contact and take overall responsibility within the team. Your tasks include outpatient visits, phone consultations, home visits, and regular rounds with doctors. You work in close collaboration both within the team and also with others such as Finspång's municipality and Rehab Finspång, where your common goal is to create safe and secure care for the patient.

About You

We are looking for you who are a specialist nurse in geriatrics, palliative care, or district nursing. Experience working in primary care, in teams, and with patients with multimorbidity is an advantage.

Application and Employment

We offer a permanent position of 100% and 40 hours/week with a start date according to agreement. Individual salary setting applies.
