Diabetes Nurse Wanted at Läkargruppen Vårdcentral

Läkargruppen Vårdcentral is a Praktikertjänst-driven healthcare center with approximately 13,000 registered patients, located in central Västerås.

We are a well-staffed and well-functioning clinic with experienced doctors and nurses, and we are now looking for an engaged diabetes nurse to join our team. The position is divided, with about 60% being diabetes-related tasks and the remaining 40% consisting of regular district nurse duties at the healthcare center.

Responsibilities

• Clinic work with patients with type 2 diabetes
• Follow-up according to national guidelines and work in the National Diabetes Register (NDR)
• Patient education, self-care support, and lifestyle counseling
• Close collaboration with doctors and other professionals
• Regular duties at the healthcare center, such as:
• Telephone counseling
• Clinic work
• Other district nurse tasks as needed

Qualifications

• Licensed nurse with further education as a diabetes nurse
• Alternatively, a district nurse with experience in diabetes care
• Experience working in a healthcare center is required
• Accustomed to independent work in primary care
• Good collaboration skills and a patient-centered approach
• Experience with electronic medical records in primary care is an advantage
