Job Responsibilities

We are looking for someone who is passionate about patient collaboration. The coordinator position serves as support not only to the operations but to the entire administration regarding processes and technical issues.

You will lead and coordinate training in the system for employees within FSF in Mina planer and related processes. A large part of the work involves following up on the use of the system in the administration's various operations and providing additional guidance as needed.

You will be a leader in the project management group between social administrations and work for close collaboration with corresponding project leaders in ASF and HVOF. Participating in various networks such as the “Network Group for Collaboration at Discharge between Region Skåne and the municipalities in the southwest” and the reference group for Mina planer is part of the collaboration.

In this role, you will have an overview of the work being done in the IT system Mina planer. You will gather input from colleagues about the system and the processes in the system (SVU, SIP, acute sheets, and soon ViSam). You are responsible for planning your work independently based on dialogues about goals and needs with your immediate supervisor.

Together with the operations, we are developing our team-based working method to provide the best possible support for the patient. You will collaborate with health and medical staff, case managers, and managers. You will also collaborate with regional and private primary care and inpatient care from a holistic view of the patient. You will drive improvement work to enhance our operations and collaboration with other actors around the patient.

Qualifications

We are looking for someone who is either an occupational therapist, physiotherapist, nurse, or case manager. It is an advantage if you have experience in municipal health and medical care. To thrive in this role, you should be attentive, solution-focused, good at collaborating, building relationships, and creating new networks. We value strong organizational skills and a high level of commitment to people's well-being and the opportunity for a dignified life. You have a good ability to engage with colleagues, section managers in the administration, and other care providers. Your approach is naturally good, and you share Malmö city's values by treating every person with understanding and respect.

You have very good experience working in the IT system Mina Planer as well as good experience with other IT-based journal systems, and you can express yourself well in Swedish both orally and in writing.
