About Us and Your Responsibilities

The ALMA team is an acute geriatric mobile team tasked with reducing hospital admissions and readmissions, providing care for our elderly patients who often feel better receiving care in their homes. We operate from Nyköping Hospital and serve the municipalities of Nyköping and Oxelösund. We admit patients via ambulance, primary care, and home healthcare to stabilize patients who have deteriorated and are stable enough to remain at home. We also follow up on patients from the emergency department who do not need admission but require close medical follow-up to be able to return home, and we follow up on patients from departments at Nyköping Hospital to reduce hospital stay and minimize the risk of readmissions.

In the ALMA team, nurses and doctors work closely together, but as a nurse, you will also make home visits alone and need to feel secure in your professional role. You will make assessments, perform checks, EKGs, blood sampling, medication management, administer medication intravenously, set up IVs, etc. An important part of the job is to see the whole picture of the patient, and therefore collaboration with other healthcare providers such as home healthcare, case managers, and home care services is essential to ensure the patient feels as safe and comfortable as possible.

The acute geriatric department has 12 beds divided into 2 teams where nurses and nursing assistants share responsibility for 6 patients. Geriatrics is a vast field, and we encounter patients with many different diagnoses, considering that we care for multi-sick elderly patients, so a holistic view is crucial. Teamwork is also an important factor here, and we have weekly team rounds to plan care goals together with the patient's needs in focus. As a nurse, your responsibilities include medication administration, nursing care, central access, wound care, blood transfusions, and mentoring students. With us, you will have excellent opportunities to develop and vary your tasks in collaboration with the entire team.

We also have several appreciated support functions such as discharge coordinators, care leaders, and meal hosts.

You will alternate between departmental work and ALMA.

Your Qualifications

We are looking for a licensed nurse. We see that you have a special interest in geriatric patients and understand the importance of a holistic perspective. You have at least a few years of experience as a nurse and feel secure in your professional role. We place great emphasis on personal qualities. A valid driver's license (B) is required.

Employment Form

Permanent employment at 100% divided between ALMA and the acute geriatric department. Weekend work is included in the duties at the geriatric department. As a nurse in the ALMA team, you will work daytime hours. Start date by agreement.
