Description

The SkaS mobile Primary Care Team offers you as a doctor a unique opportunity to try out an exciting assignment.

You will combine your regular position with your new assignment. The setup means that you will rotate into the Primary Care Team together with two to three other doctors.

The SkaS mobile Primary Care Team is the hospital's extended arm and works in the homes of patients throughout Skaraborg. The assignments of the Primary Care Team come from all care units and the emergency department of SkaS, pre-hospital care, the Healthcare Alarm Center, as well as regional and municipal primary care. The operations are assignment-based with the aim of increasing the conditions for specialized care at home, reducing unplanned inpatient care and emergency visits, or replacing inpatient care.

As more care can be managed outside the hospital, we are now looking for you who want to help develop the Primary Care Team as an alternative form of care.

Responsibilities

In your role as a doctor in the Primary Care Team:

• you will be responsible for the medical care
• you will assess and plan assignments where home care can replace or shorten hospital stays
• you will conduct home visits together with nurses or via remote contact
• you will collaborate with colleagues, care neighbors, and other care units daily.

You will work together with a well-functioning group of nurses from different operational areas in the hospital and in ambulance services. We work together to provide patients with safe and secure care at home.

Qualifications

Swedish medical license with specialist competence.

Competence and experience in internal medicine, cardiology, geriatrics, or general medicine is an advantage. Other specialties are also welcome to apply as great emphasis is placed on personal suitability.

You have an easy time collaborating, possess good communication skills, and have a holistic view with the patient in focus. We value the ability to collaborate well, as the work involves many contact points with care neighbors and colleagues.

You have an interest in participating in development work to improve care for the residents of Skaraborg.
