JOB RESPONSIBILITIES

Do you enjoy working closely with patients to provide them with quality and service? We are now looking for two skilled nurses for Sätra Home Care. With us, you will have a fun and flexible job where we work together for a holistic view of the patient.

As a nurse in home care, you will play a central role for our admitted patients. You will work both independently and in a team. You will be responsible for the care of several patients admitted to home care. As a nurse, you will perform health and medical tasks in the patient's home, working preventively to promote the patient's health and well-being.

Your tasks include:

• Performing health and medical interventions for patients in home care
• Being responsible for care and conducting medical assessments
• Working in a team around the patient with, among others, physiotherapists, occupational therapists, and doctors
• Planning and conducting home visits and follow-ups
• Guiding and delegating care tasks to nursing staff and students
• Documenting in Treserva and registering information in other operational systems
• Working in our consulting operations during evenings and weekends

We have a close collaboration with the palliative team of Region Gävleborg, which is an important assignment. As a nurse, you have a great responsibility to contribute to the continuous development and improvement of the operations.

The position is primarily daytime work, but the schedule includes evening and weekend shifts. You will work in our consulting operations, which include the municipality's care and nursing homes, home care, and service and group homes within LSS and social psychiatry.

QUALIFICATIONS

You are a confident and structured nurse with a genuine interest in patient-centered work. In this role, you have a central function for our admitted patients in home care. You will work both independently and in close collaboration with other professionals in a team.

• Licensed nurse
• At least two years of experience as a nurse
• Have a B driver's license and experience driving a car
• Have documentation experience
• Good knowledge of the Swedish language in speech, writing, and reading comprehension

It is an advantage if you have experience in municipal health and medical care and if you are familiar with the operational system Treserva, which is our documentation system.

To succeed in this role, you are:

• Confident in your role as a nurse.
• Flexible and can quickly adapt and make decisions based on the patient and their needs
• Proactive and take responsibility for ensuring your tasks are completed
• Structured, good at planning
• Able to prioritize and be efficient, using resources to achieve results in the best way
• Have good communication skills and adapt your language according to the recipient

Documentation is done in Treserva, planning is done in TES. Lifecare is used for communication between care providers.
