Your new job

The staff in the Home Team meet individuals in their homes when they return from the hospital. Our work is characterized by a health-promoting, flexible, and solution-focused approach with the individual's needs at the center. As a care assistant in the Home Team, you perform care and nursing tasks based on a supportive and rehabilitative approach. We operate from the Home Team's premises and work in the individual's home. Our goal is for the individual to have the opportunity to influence and take responsibility for their own situation. We independently plan the work based on the individual's conditions and needs. As we are an assessment team, we work a lot with evaluations and have a distinct rehabilitative approach.

You provide good and safe, person-centered care and assess what care, nursing, and rehabilitation needs exist. You establish implementation plans and update them with current needs, and you work based on a rehabilitative approach. The interventions are usually carried out over a two-week period, after which they are handed over to the regular home care group or the citizen may not need further assistance.

In your work, you will collaborate closely with case managers, occupational therapists, physiotherapists, nurses, administrators, and unit managers.

Your qualifications

We are looking for someone who is a trained care assistant, and it is advantageous if you have experience in care work. To thrive in the role of a care assistant in the Home Team, we believe you are proactive and take responsibility for your tasks. You remain calm in stressful situations and can easily adapt to changing circumstances. Furthermore, we believe you work well with others, are outgoing, and communicate smoothly in your work. This is necessary as you will have a lot of contact with the individual, relatives, and other staff. You should also be interested in development work and be able to express yourself well in Swedish, both spoken and written, and have good reading comprehension. We also welcome other language skills.

A manual driving license (B) is required for the position.

Welcome to us

The Home Team consists of care assistants, nurses, occupational therapists, physiotherapists, and case managers. Together with the citizen, the Home Team plans the care and support that the citizen needs during an assessment period in the individual's home. The Home Team operates from Klubbhusgatan 13 in Jönköping. The workgroup consists of about 30 care assistants, but a total of about 70 employees work together at the workplace. Good to know

Within Jönköping Municipality, we offer equal pay for care assistant positions regardless of which elderly care facility you work at. This means that the salary is not affected by the choice of workplace. All our elderly care facilities have comparable working hours, and we follow current legislation, including rules on rest periods and weekend work.

The recruitment refers to a permanent position where the work is scheduled for days and evenings, both weekdays and weekends. Start date according to agreement. We work with joint planning, which means that work at several units will be relevant. You will work every other weekend with us. Jönköping Municipality strives for full-time as the norm and always offers a 100% employment rate.
