About Us

The Psychosis Clinic West is an outpatient clinic for individuals with psychotic disorders within Sahlgrenska University Hospital.

We are a multidisciplinary team with high competence. The clinic employs doctors, nurses, caregivers, psychologists, social workers, occupational therapists, peer support, treatment assistants, and medical secretaries.

The treatment method we work with is called Resource Group ACT (R-ACT), which is a model for coordinating care and support interventions in psychosis care. The person working with R-ACT is called a case manager and is responsible for coordinating, investigating, planning, implementing, and evaluating care.

We focus on close collaboration with the patient and their network, such as family, housing staff, or representatives. Together with the patient, we work for increased influence over care and treatment as well as the patient's own goals.

Job Responsibilities

This is a versatile and varied assignment where you plan your tasks as a Case Manager. You play a key role in contacting the patient and collaborating with the patient's network such as inpatient care, social services, and housing support. Our clinic has an established collaboration with housing services in Gothenburg and social services. As a social worker, you are tasked with monitoring the perspective of children, providing support to relatives, and conducting grant applications, among other duties. Other common tasks include contacts with authorities, financial support, and applications for guardianship or trusteeship.

About You

We are looking for someone who has completed a degree in social work and has experience working in psychiatry.

Previous experience in psychosis care and R-ACT is an advantage. You should be able to perform the work independently and efficiently. Additionally, it is desirable if you have knowledge of and can navigate Melior and Elvis.

We are looking for someone with a positive outlook on life who works solution-focused. A driver's license is a bonus as we conduct home visits.
