The elderly care consists of three geographical areas: Svalöv, Teckomatorp, and Kågeröd. Within each area, there are special accommodations and home care. Here, nurses, occupational therapists, physiotherapists, care assistants, and rehabilitation assistants work in teams based on the special accommodations.

Svalöv is part of the municipal association Medelpunkten, whose mission is to meet the needs of member municipalities for the aids that the municipalities are responsible for.

Together with the healthcare center Svalöv, home healthcare has a home healthcare team, which means that patients living in any of our special accommodations for the elderly, LSS group homes, or receiving home healthcare from a nurse are offered good access to doctors. The collaboration is close, and we continuously work to develop our home healthcare team to provide close and good care to our patients.

JOB RESPONSIBILITIES

You will work with home healthcare for individuals living in special accommodations for the elderly, LSS group homes, social psychiatry housing, or in regular housing. You have a central role in collaboration with and around the patient and ensure good quality in care. Documentation is done in our operational system Lifecare HSL.

The physiotherapist's responsibilities are to independently and/or together with other professions in the team strengthen the individual's independence and increase functions and participation in daily life. Your work includes conducting functional assessments, prescribing aids, as well as rehabilitation and habilitation together with the patient. You assess, plan, follow up, and evaluate the healthcare that the patients need.

You also provide support and advice to care staff, delegate healthcare tasks, and lead efforts within your area of expertise.

The workday consists of planned interventions and home visits; sometimes unplanned events arise that require you to be able to reorganize your work. The work is independent and flexible, and you are involved in developing a care and support system we can all be proud of.

QUALIFICATIONS

We welcome all licensed physiotherapists to apply. It is advantageous if you have experience from municipal primary care, are confident in your professional role, have the ability to collaborate, and can independently plan and prioritize your work.

You must have good computer skills and be proficient in the Swedish language, both spoken and written.

Great importance is placed on personal suitability and good references from previous workplaces.
