We are looking for a diabetes nurse who wants to help develop HMC in Hjärup

The Health Medical Center in Hjärup is part of the HMC group, which operates health centers, child health care, maternal health care, and occupational health care in Hjärup, Lomma, Bjärred, Landskrona, Arlöv, Malmö, and Staffanstorp.

We started in Hjärup in 2009 and currently have about 9000 registered patients.

Hjärup is a community with about 4500 inhabitants located between Lund and Åkarp. There are Pågatåg trains, good bus connections, and free parking at HMC.

About the position

The position involves being responsible for and developing the diabetes clinic at the health center in close collaboration with the rest of the team. The work is largely independent, which means you should be able to make your own assessments and decisions. You will have a multidisciplinary team at your disposal, close cooperation with the responsible physician, regular quality rounds, and a network with other diabetes nurses in our organization.

The position also involves participating to some extent in the health center's other work such as telephone advice, nurse consultations, and vaccinations.

About you

We are looking for a licensed nurse or district nurse with further education in diabetes care. We prefer that you have good experience with documentation in the PMO medical record system.

Furthermore, we would like you to also

  • have experience in primary care

  • have good knowledge of the Swedish language, easy to communicate and build relationships

  • be confident both personally and professionally

  • be driven to meet the patient where they are and have a desire to take responsibility for the entirety of the patient's care experience

  • strive to guide your patients towards sustainable living habits

  • appreciate a smaller organization with short decision-making paths and a flexible working method through close real-time collaboration with a multidisciplinary team

  • want to be part of a fantastic team of happy and competent colleagues

Scope: Full-time

Employment form: Permanent employment, we apply a 6-month probationary period

Start date: By agreement
