Job Responsibilities:
- You support clients in their daily lives, all according to the social worker's decisions.
- Provide support and companionship, and help create a meaningful everyday life.
- Use our digital planning system to plan the clients' services.
- Document in the operational system and follow individual implementation plans.
- Through our contact person system, you will have special responsibility for some clients, where you help create and follow up on implementation plans and manage information transfer.
- Collaboration is key! You work closely with other home care teams and HSL staff to ensure that all clients receive the best possible care.

Qualifications:
We are looking for someone with education in care and nursing or equivalent experience, as well as good knowledge of Swedish, both spoken and written. You have the ability to work independently and take initiative, as well as the ability to plan and organize your work. We also place great importance on your ability to collaborate. A valid driver's license (B) is required.
Conditions:
The current position includes day, evening, and weekend shifts. Start date 260202 or by agreement. The employment is a permanent position. For employment, a copy of the criminal record for HVB homes (in a sealed envelope) is required.
