The adult psychiatry area in Lund provides specialist psychiatric healthcare for adults over 18 years old. We conduct assessments/investigations of psychological symptoms and issues and offer care, treatment, and rehabilitation based on a multidisciplinary competence. Our catchment area includes eight municipalities in central Skåne: Lund, Lomma, Burlöv, Eslöv, Hörby, Höör, Kävlinge, and Staffanstorp.

One of the biggest challenges in healthcare and social services is to ensure continuity, wholeness, and participation in the range of care and support for the target groups that need it the most. For individuals with severe mental illness and disorders, we have long known that structured collaboration forms are required for this to be possible. The national guidelines from the National Board of Health and Welfare prioritize integrated coordinated efforts for individuals with serious psychiatric issues. One of these collaborative models is Flexible Assertive Community Treatment (FACT), and an implementation of the FACT model is underway in various ways throughout Skåne. A success factor for the implementation work in central Skåne has been the support from a process leader appointed since August 2022, who has kept the work together in the sub-region.

Within the framework of the psychiatric collaboration in central Skåne, we now want to continue developing the work with FACT so that the methods and efforts benefit all residents with such needs. The goal is to be able to work with FACT as a working method in a similar way for all users in central Skåne who belong to the target group.

JOB RESPONSIBILITIES

As the current process leader is moving to another assignment, we are now looking for a process leader who can continue to lead and coordinate this work, in close collaboration with all municipalities in the sub-region and Region Skåne. You will be employed by Region Skåne but will have a collaborative assignment based on the psychiatric collaboration in central Skåne, where all municipalities are represented. The municipalities in central Skåne are responsible for taking an active role in implementing FACT in their municipality.

In the role of process leader, you will work closely with both the municipalities and the specialist psychiatric units within Region Skåne to continue developing the initiated FACT collaboration. You will have a driving role as implementation support for the Lund Addiction Reception, which is the latest of our receptions to implement the collaborative model.

Your responsibilities include developing common routines and guidelines as well as any agreements for the FACT collaboration. Additionally, you will plan for and/or participate in training and inform about FACT in the sub-region. You will support the municipal coordinators within the collaborative model and maintain a sub-regional network for these.

The position is a project employment for two years. Your place of work is Lund, but travel may occur in the role.

QUALIFICATIONS

We are looking for someone with education in healthcare or other post-secondary education that the employer deems equivalent. You have good knowledge of Swedish in both spoken and written form. Furthermore, you have good computer skills and knowledge of the Microsoft Office suite. Experience of collaborative work between regions and municipalities is an advantage. If you have experience in project management and/or managerial experience, it is beneficial. Additionally, experience in meetings with individuals within the target group of psychosis and substance abuse is also meritorious.

We are looking for someone who has a genuine interest in the target group and development work. As a person, you are structured and organized and used to working independently. Since you will work closely with employees, managers, and representatives from municipalities, you need to have good communicative skills and enjoy collaborating and cooperating with others. Furthermore, you have the ability to work from a holistic perspective for different processes. We place great importance on personal suitability.
