In the field of psychiatry and habilitation, the regional healthcare includes adult psychiatry, child and adolescent psychiatry, and habilitation in Region Västernorrland. The regional healthcare services are provided at three hospitals: Sundsvall, Sollefteå, and Örnsköldsvik, as well as at several clinics located in various towns in the region. The area has approximately 700 employees.

The Addiction Team is a regional team stationed at the psychiatric clinic in Sundsvall. The team acts as a link between specialist care, primary care, and the seven municipalities in the region. Our mission includes assisting healthcare and social services with consultative support and being a knowledge center regarding harmful use, addiction, and comorbidity.

We now have the opportunity to expand the team with a position, and we aim to strengthen the clinic's collaboration both internally and externally in the work with psychiatric disorders and comorbidity. Therefore, we are looking for a social worker who will primarily work with SPOT (Specialist Psychiatric Care Team) and the target groups of the needle exchange program, as well as in the regional development work together with us.

SPOT is a psychiatric care initiative that is carried out on behalf of doctors within Specialist Psychiatry. The team is one of three in Västernorrland and is staffed by nurses, doctors, treatment assistants, and caregivers with extensive experience in psychiatry.

The care services offered are based on the patient being at home, where the level of care lies between traditional outpatient care and inpatient care. SPOT is a mobile resource that provides support services for a wide range of psychiatric health conditions and is part of psychiatry's initiative to prevent admissions, reduce lengths of stay in inpatient care, and offer patients more participation and close care.

We are now looking for a social worker to join our team! The position is a permanent full-time employment with a start date according to agreement.

Social Worker for the Addiction Team, Psychiatric Clinic, Sundsvall

Job Responsibilities

• Patient work in the form of coordinating efforts and collaborating with other care providers around the patient
• Conduct psychosocial assessments/evaluations
• Home visits, telephone counseling, clinic visits, or video visits with patients are included
• Working with SIP (coordinated individual plan) is central
• To be updated on community support functions aimed at individuals with psychiatric issues (e.g., from municipalities, social insurance, employment services, etc.)
• To pay attention to children's needs and the need for parental support in work with adults
• Refer to family support
• Participate in rounds/conferences and offer psychosocial counseling in patient matters to other colleagues
• The position includes work within the needle exchange program.
• Drive the team's efforts to build collaboration with the region's seven municipalities

We are looking for someone who has a good ability to create supportive relationships with patients, partners, and relatives. Having a well-developed ability to work independently as well as together with other professional categories is an important success factor.

• Has a degree in social work or another relevant education that the employer deems equivalent
