Hospital-Connected Home Care (LAH) in Mjölby
Hospital-Connected Home Care (LAH) in Mjölby is part of the Western Nursing Clinic, which is included in the primary care center in western Östergötland.
The operation runs around the clock and offers specialized home care as well as palliative care in the patient's own home. As an employee at LAH, you meet people of different ages, with various diagnoses and significant medical needs. Our catchment area includes Mjölby, Boxholm, and Ödeshög municipalities. At night, we work together with our colleagues at LAH in Motala across the entire western part of the county, which also includes the municipalities of Motala and Vadstena.
In addition to primarily working within the home care unit in Mjölby, you will also, IF NEEDED, assist in staffing our inpatient care unit, ward 11, which is physically located at the Hospital in Motala.

Job Description
We are now looking for a nurse for a permanent position with a rotation service as we are a 24/7 operation.
About You
We are looking for you who are a licensed nurse and preferably have previous experience and interest in home care and palliative care. You must be proficient in Swedish, both spoken and written.
As you work in constant dialogue with patients and colleagues, it is a requirement that you have good knowledge of the Swedish language, both spoken and written.
Specialized home care sometimes involves difficult encounters and also difficult decisions in the patient's home, and therefore we are looking for someone who has personal maturity, is self-sufficient, and flexible. As a person, you are positive and see opportunities instead of difficulties. Since part of the palliative care philosophy is teamwork, it is also important that you are a team player. You should have a good attitude towards both colleagues, patients, and relatives. You focus on the patient's well-being and always strive to do your best.
