Who we are
Stockholm's Dementia and Special Team is a private home care company since 2010 that provides services in the customer's own home based on assistance decisions. We primarily cater to customers with dementia diseases and mental/physical disabilities.
We provide: traditional home care, housing support, escort services, respite services, household services, and additional services, and you will be responsible for all parts.
We operate in Södermalm, Liljeholmen-Älvsjö-Hägersten, Norrmalm, Östermalm, Kungsholmen, Skarpnäck, Enskede-Årsta-Vantör, Skärholmen, and Farsta, and our office is located in Södermalm where the service will be.
The organization includes a Operations Manager, Deputy Operations Manager, Administrator, and Team Leader.
About the position
Probationary employment at 100% Monday to Friday from 08:00 to 16:45.
Please note that there is no opportunity for flexible hours or to work from home as your primary task is to be present at the organization's office for staff and customers.
Who are we looking for?
We are looking for someone with a strong sense of responsibility, who can work independently, is meticulous in their work, solution-focused, stress-resistant, and can work in a team.
You have held a coordinating position in a management team within home care before and have been responsible for staff, customers, and part of the daily operations. The role means that you will have both fixed and flexible tasks with a large degree of responsibility and be able to develop together with your colleagues in the office for common tasks.
As you will have close contact with staff, customers, relatives, and caseworkers, it is of utmost importance that you have interest, understanding, and knowledge about the customer orientations the company has (dementia and mental health).
Your background:
Knowledgeable team leader/coordinator with several years of experience in home care.
Very good Swedish in speech and writing.
Have worked based on assistance decisions within home care and housing support.
Good knowledge of memory diseases and mental health as you will only work with these orientations and need to guide staff in this as well as have a high understanding of the organization's customers to perform a professional job.
Customer responsibility.

Some occupational health and safety responsibility.
Be computer literate and quick on the keyboard.
It is an advantage if you are familiar with the data systems Parasol/Paraply, Carefox, and Paragå.

Education:
It is an advantage if you are a trained social worker or equivalent and have completed training in personnel administration, leadership training, occupational health and safety training, etc.
Generally, the role of team leader at Stockholm's Dementia and Special Team means that the employee ensures quality and monitors the daily work within home care and housing support. The team leader will collaborate and report to other members of the management team according to the organization's routines. In your position, you will be responsible for several districts where customers and staff are included.
Selection of areas of responsibility:
Staff responsibility:
• Staffing responsibility.
• Guide staff towards customers in the field, by phone, and through personal conversations in daily work.
• Investigate, have conversations, and follow up on complaints and feedback.
• Ensure quality and monitor staff work and ensure they follow routines.
• Responsible for the introduction and follow-up of new employees.
• Hold weekend and evening meetings with staff.
Customer responsibility:
• Main responsibility for new and existing customers.
• Home visits, follow-ups, and risk analyses/assessments.
• Handle assistance decisions and standard calculations.
• Enter service times and customer information and update in the organization's scheduling system.
• Establish implementation plans together with the customer/contact person and enter into Stockholm city's data system.
• Ensure and arrange that keys, powers of attorney, and personal funds are available.
• Establish and update customer records.
• Review assistance decisions and follow up documentation.
• Investigate, have conversations, and follow up on complaints and feedback.
• Have contact with external companies such as subcontractors, health centers, and occupational therapists regarding customers.
• Have contact with relatives and guardians.
• Have close contact with caseworkers regarding customers' assistance decisions and report changed services.
• Write care reports as needed.
• Report incidents and deviations in Stockholm city's data system.
• Hold SIP meetings with customers, relatives, and caseworkers.
• Investigate and follow up on complaints and feedback regarding customers.
Other:
• Be helpful via phone and email for staff, customers, relatives, and external organizations.
• Attend networking meetings with external organizations and the elderly administration.
• Perform self-checks and risk analyses according to the organization's routines.
• Have shared responsibility for occupational health and safety work.
