Job Responsibilities

We are looking for you who want to work as a care planning nurse in the care planning team in Hisingen.

Our office is located at Lillekärr Södra 51. You will work Monday to Friday, daytime with the possibility of flexible hours.

As a care planning nurse, you will be part of a cross-professional care planning team in municipal primary care consisting of nurses and rehabilitation specialists. You will work based on the health and medical care process of the City of Gothenburg and the health and medical care agreement for assessment, decision-making, and planning of services after inpatient care for patients who are to be admitted to municipal primary care. The care planning team participates in planning meetings and SIP (coordinated individual plan) together with social workers from the authority, inpatient care, and regional primary care and follows current guidelines for the admission and discharge process.

In the care planning team, you will also assess the need for municipal health and medical care services based on the current health and medical care agreement for non-admitted patients where care requests are received. You will investigate care requests from regional primary care and inpatient care and together with the care planning team make a decision about admission to municipal primary care. From a patient-focused perspective, a safe care and care planning is established where everyone in the team participates for the patient's best and creates a safe and independent daily life for the patient. Collaboration and coordination are a major part of the work.

As a care planning nurse, you are expected to take a leading role where you support your colleagues in the unit regarding the health and medical care agreement, admission and discharge processes, and self-care assessments. You have an important role where you actively participate in the group to increase knowledge about SIP and participate in networks and forums for collaboration with both inpatient care and regional primary care to create good collaboration in close care. Qualifications

We are looking for you who are a licensed nurse and passionate about making a good and safe plan for the patient in connection with assessment and admission to municipal primary care. We prefer that you have experience in nursing and/or experience in care planning. You have good computer skills and a very good ability to express yourself verbally and in writing, as the work requires documentation. A B driver's license is also a requirement as you may need to drive to patients in some cases.

As a person, you have good self-awareness and feel secure in your work role. You find joy in contributing to good care for patients and relatives. The profession of a nurse in the care planning team involves many meetings with different people. Therefore, we see that you should have the ability to create trusting relationships, be attentive, and at the same time clear in your way of communicating. You enjoy working in a team and with your enthusiasm and joy at work, you contribute to the success of the workgroup. You can make decisions and structure and plan both your own and others' work.
