About the position
In our internal medicine department, we care for patients with nephrological and endocrinological diseases. We have both planned and acute admissions. Our nephrology focus is unique as we are the only department in Southern Stockholm performing peritoneal dialysis. Therefore, you will gain specialized skills in performing peritoneal dialysis manually and with machines. Our patients may also require surgical interventions during their stay with us, which means you will also gain experience and knowledge in surgery and orthopedics. Our goal is to help and support individuals living with lifelong treatment and chronically ill patients to maintain or improve their quality of life. Our patient flow in endocrinology includes, among other things, patients with newly diagnosed diabetes or diabetics with complications such as diabetic foot ulcers or ketoacidosis. A cornerstone of our operations is working in teams together with patients and their relatives to provide the best care with the patient at the center.
We are characterized by a lack of pretentiousness and have close collaboration between different health professionals as well as partners within and outside the hospital. As a Coordinator, you will have your own responsibility in leading the planning of our patients who need support with planning and measures before discharge. We are looking for 1-2 colleagues to work 50-100% in the coordination task.
The position involves working clinically and contributing to our 24/7 operations by working weekends. How often depends on whether we hire 1 occupational therapist or nurse/assistant nurse.

- understands the various resources available for patients, both in inpatient care (e.g., occupational therapists, dieticians) and at home (e.g., home healthcare, social services)
Qualifications
Requirements:
- Licensed nurse or
- Licensed occupational therapist or
- Assistant nurse
- Experience coordinating discharges to other healthcare providers
- Experience working in Lifecare or similar
Desirable:
- Experience in nephrology, endocrinology, dialysis, or diabetes
