Within the LSS unit, our mission is to provide basic health and medical care to individuals living in group or service housing who participate in daily activities. We strive to create a safe and supportive environment for our patients.
In the unit, we work together as a dedicated team. There are four nurses, three occupational therapists, and two physiotherapists, and in the fall, we plan to recruit more staff to meet our expanded mission. We operate from our office in Artursberg. During the day, a large part of our work takes place directly with our patients, either in their homes or at their daily activities.
We apply a person-centered approach where we perform and follow up on prescriptions and conduct individual assessments. A central part of our work is to support and guide the care staff who work closely with our patients. With us, you will have a varied workday with great diversity and the opportunity to use your experience and work independently, while always having colleagues to consult with when needed. Your tasks

Your tasks as a nurse in the LSS unit involve being responsible for a number of patients in the different units where collaboration with primary care and specialist healthcare is often part of the work. You will be part of our interdisciplinary team that works around the patient.
A significant part of your role is to identify preventive measures and implement them in collaboration with the care staff, either in the patient's home or their daily activities. Together, we create a safe and supportive environment for our patients.

A day at work

As a nurse in the LSS unit, you will play an important role with us. The work includes, among other things:

- Providing health and medical care up to the level of a nurse.
- Ensuring that medication management is carried out safely, including through delegations.
- Guiding and instructing care staff in the operations.
- Quality assuring health and medical interventions, e.g., through registration in quality registers.
- Participating in development work.
- Collaborating in the interdisciplinary team around the patient.

Who are you?

You are confident in your professional role, work efficiently and flexibly with the ability to structure and organize your work both with patients and in the office. As a nurse within the LSS unit, it is important that you possess a broad knowledge and good communication skills.

Requirements:
- Licensed nurse, at least 2 years of experience.
- Experience in municipal health and medical care and/or home healthcare.
- Ability to express yourself well in Swedish, both orally and in writing.
- You have good documentation skills and good IT skills.
- B driver's license.

Meritorious:
- Specialist trained in district care, psychiatry, or other training relevant to our target group.

Personal qualities:
- Ability to work independently and be unpretentious.
- Through your engagement, you create good relationships.
- A genuine interest in working with people.
- Ability to lead and motivate, find solutions and see opportunities.
- Enjoy working in interdisciplinary teams.
- Like to guide care staff and be a support and security in health and medical issues.

We offer you

Our team within the LSS unit is a wonderful and close-knit group that looks forward to welcoming new colleagues. The working hours are during the day, weekdays between 07:00-15:30 (including public holidays that fall on a weekday).
We are now looking for two full-time nurses for a fixed-term period starting immediately until August 31, 2025.
You will be employed by the health and medical care unit in Södertälje municipality with a focus on LSS. If the needs of the operation change, it may be relevant to be placed in another area.
