We are looking for a district nurse or registered nurse who wants to help build up home care in Södertälje.

We are now looking for someone who wants to become part of our team. We value a flexible and close collaboration among all our professional groups to best assist our patients.

The position is full-time and scheduled on weekdays during daytime. The unit is located in the center of Södertälje with a view over the canal and good connections with both commuter trains and buses.

Your roleTogether with nursing assistants, you will perform planned and acute health care interventions in our patients' homes. Your responsibilities as a district nurse/registered nurse in our home care team include medical assessments, handling of medical technical products, medication management, and supervision of colleagues, patients, and their relatives.

You have a central role in collaboration around the patient where you ensure a high quality of care and follow-up of the patient. You will be close to decision-making, both within your own team and with managers. QualificationsWe are looking for someone with at least two years of experience in home care, geriatrics, or from assisted living facilities. We expect you to have experience and an interest in working on improvement and development within patient safety and to be environmentally and cost-conscious.

You communicate fluently in Swedish both verbally and in writing and have good computer skills in both medical record systems and Office 365; we use the TakeCare medical record system. A driver's license is required.
