Vårdcentralen Landborgen is a modern health center that is currently Helsingborg's largest health center, located in the heart of the city with proximity to the center, forest, and sea. We have a high level of trust from our approximately 13,600 registered patients.

The unit currently consists of 57 enthusiastic employees with a good work climate. Here, we strive for good care with continuity and teamwork within and outside professional boundaries. We are a well-staffed health center with specialists in general medicine, AT and ST doctors, district nurses, registered nurses, nursing assistants, medical secretaries, dietitians, social workers, occupational therapists, rehabilitation coordinators (REKO), and physiotherapists. We have well-developed specialist clinics in diabetes care, asthma/COPD, dementia, elderly care, and lifestyle.

With us, we work in interdisciplinary round teams where all professions meet twice a week to discuss patient cases. This creates continuity, security, competence supply, and job satisfaction. We place great importance on having a good work environment with variation in work and the possibility of individual schedules.

JOB RESPONSIBILITIES

We are looking for someone with a licensed profession who wants to work as a rehabilitation coordinator. With us, you contribute to taking good care of others and helping people feel better.

Here, you are the patient's individual support in the sick leave process, where part of the work involves mapping the patient's needs, conditions, and personal goals. The mapping can then form the basis for the rehabilitation plan that is made together with the sick-leaving doctor. Here, you also include the patient's own wishes in the plan. Another task is to collaborate with other authorities. As a rehabilitation coordinator, you are the healthcare unit's contact person for municipalities, employers, the Social Insurance Agency, and the Employment Agency in sick leave matters. Additionally, you are also responsible for internal coordination. This may involve keeping track of when sick leaves expire during the doctor's absence or contacting other parts of the healthcare system that are also relevant to the patient. Statistics and analysis of sick leave patterns are also part of the internal coordination.

QUALIFICATIONS

We welcome you who have a licensed profession with the authority to operate within Swedish healthcare, for example as a nurse, occupational therapist, or physiotherapist. Language skills in Swedish at least corresponding to level C1 according to the Council of Europe's level scale are a requirement.

As a person, you enjoy working with a patient-focused approach with a holistic view and a care chain perspective. You have the ability to work independently and under your own responsibility while collaborating well in a team. Additionally, you have good pedagogical skills and the ability to take initiative.
